2017 State Convention Registration
Please fill out this form to register for the 2017 DPW State Convention on June 2nd and 3rd in Madison.
The early registration deadline has now passed. The fee to attend is $35. You can register online up until 5pm on Monday, May 29th. After that, all registrations must be at the door, where the price will be $35 as well.
If you are paying for someone else or for more than one person, you will also need to enter the additional names at www.wisdems.org/conventionsignup
Contribution rules
- I am at least eighteen years old.
- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am making this contribution with my own personal credit card and not with a corporate or business credit card or a card issued to another person.
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
