- I am at least eighteen years old.
- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
Paid for by the Illinois Democratic County Chairs' Association, PO Box 3445, Springfield, IL 62708-3445, which is a committee not authorized by any federal candidate. However, please note your contribution may be used in connection with a federal election. Federal law requires political committees use their "best efforts" to report the name, mailing address, occupation, and name of employer for each individual whose CONTRIBUTION AGGREGATE IN EXCESS OF $200.00 IN A CALENDAR YEAR.
