- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).

A copy of our report filed with the State Board of Elections is or will be available on the Board’s official website (www.elections.il.gov) or for purchase from the State Board of Elections, Springfield, Illinois. Contributions and gifts are not deductible as charitable contributions for federal income tax purposes.
