  1. I am not making this contribution on behalf of a corporation, LLC, or union.

  4. This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.

You may contribute up to $6,650 per election cycle to an Arizona Maricopa county candidate. Arizona law requires the candidate to report the name, occupation, and employer for any out-of-state contributor and the name, address, occupation, and employer for any in-state contributor contributing over $100. You may contribute up to $5,000 per year for each primary and general election to a Nevada Washoe county candidate. Nevada law requires the candidate to report each contribution from a contributor who cumulatively contributes more than $100 during the year, including their name and address. Your contribution to Arizona candidate, party, or political committees may be used to pay for campaign media spending in Arizona. The Arizona Citizens Clean Election Commission rules require that these committees report the identity of donors who contributed more than $5,000 of funds for campaign media spending. If you do not want your funds used or transferred for campaign media spending in that state, you may email Tim Stringham's, Aubrey Sonderegger's, Emilia Cortez's, and Joel Navarro's campaigns within 21 days of receiving this notice.
