This payment form is for those using a state campaign committee, state PAC, other state non-candidate committee, or company funds. This form also accepts personal funds. If you are using federal campaign committee, fed PAC, or other federal non-candidate committee funds, or want to donate personal funds as federal funds, please submit a donation through the federal form.
The maximum amount this form will accept is $25,000.

- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
