Please join us for the Hoosier Women Forward Annual Luncheon on Wednesday, September 25th from 11:30am-1:00pm at the Indiana Roof Ballroom in Downtown Indianapolis. Doors open at 10:45am for this event.
This form is for ticket purchases by individual donors. If you are interested in a corporate event sponsorship, or in purchasing more than 8 tickets, please contact info@hoosierwomenforward.org.

Contribution rules
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am at least eighteen years old.
Contributions to Hoosier Women Forward are not tax deductible for federal income tax purposes.
