- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).

A copy of our report filed with the State Board of Elections is (or will be) available on the Board’s official website (www.elections.il.gov) or for purchase from the State Board of Elections, Springfield. Contributions to Friends of John R Carrozza for Judge are not tax deductible.
By contributing I understand and certify that: I am at least eighteen years old. This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution. This contribution is not made by a federally chartered bank or corporation. I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder). State law requires the committee to obtain the name and address of each individual who contributes more than $150 in aggregate, as well as the occupation and employer of each individual who contributes more than $500 in aggregate. By contributing, I hereby agree to provide accurate information of same.
