End Homelessness Vermont works primarily with people living with complex medical needs and disabilities who often have the highest barriers to finally making it home. In the last few years, people have experienced endless exits to unsheltered homelessness and these have caused extensive setbacks in access to supports, services and their housing search.
Every moment of progress lost while unsheltered, means a longer time in need of homelessness supports. It is in our clients and our states best interest that we ensure that there is a seamless transition to housing from homelessness as the first and most primary need, while being careful to have a client centered approach, ensuring services are appropriately navigated, medical needs are met along the way and the systems that are supposed to help people are not harming them.
This is why we have initiated the Making It Home fund, to ensure that our neighbors will have all the support they need from the moment of emergency into permanent housing and beyond. End Homelessness Vermont has a 99.8% track record in keeping clients engaged in our services. When we don't leave people behind, we can end homelessness in Vermont.
