  2. This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
  3. I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
  4. This contribution complies with the provisions of DuPage County Code Chapter 2, Article VII, Section 2-402.
A copy of our report filed with the State Board of Elections is (or will be) available on the Board's official website (ww.elections.il.gov ) or for purchase from the State Board of Elections, Springfield, Illinois.
