TO REQUEST A $75 ($150 couple) POLITICAL CONTRIBUTION REFUND (PCR):

Step 1 - PLEASE EMAIL info@jayxiong.org stating you are requesting for a PCR RECEIPT from the Jay Xiong for MN House Committee. Please include the name(s) of the person(s) requesting the PCR in your e-mail.

Step 2 - Once you receive the PCR RECEIPT, you must fill a PCR APPLICATION. Click HERE to print your PCR APPLICATION.

Step 3 - Mail your PCR RECEIPT with the filled PCR APPLICATION to the MN Department of Revenue address provided on the PCR application.

If you prefer to mail your contribution, please make out a check to "Jay Xiong for MN House" and include your employer and occupation in the memo field.
